COMMISSIONING THAT IS ENGAGING - THE HEALTHY LIFECHECK EXPERIENCE IN LEICESTER

The Leicester CVD Screening Service is primarily a CVD 'case-finding' service, thus it refers patients with an absolute risk of 20% or greater. It is also designed to signpost patients with significant, but less than 20% risk, into appropriate public health 'lifestyle' services such as smoking cessation.

Community pharmacists have screened over 2500 patients in the first year and identified around 500 patients who met the referral criteria. The commissioning model supporting this service, which has effectively engaged pharmacy contractors, will be studied during this presentation.

ALCOHOL SCREENING AND BRIEF INTERVENTION SERVICE: The Wirral Case Study

Alcohol misuse is recognised nationally as being of critical concern in respect of health, including the rise in emergency admissions, complex cases and delayed transfers of care, as well as alcohol related violence.

The Wirral Alcohol Harm Reduction Strategy (2007-2010) was developed in accordance with the Department of Health's Models of Care for Alcohol Misusers (MoCAM) being key to a planned programme of improvement, as it focuses on categorisation of alcohol misuse. Using this categorisation it was identified that limited services were available for alcohol misuse and in particular there were no services for the hazardous and harmful drinkers on Wirral. Re-design across a range of services has been essential in addressing this increasing problem.

Wirral Pharmacies have been contracted to deliver a co-ordinated alcohol identification, screening and brief intervention programme as part of the Wirral Alcohol Harm Reduction Strategy 2007 - 2010.

Peace of mind in dispensing unlicensed medicines

The sourcing and provision of unlicensed medicines can present a number of challenges to pharmacists who are clearly keen to ensure patients receive a safe and effective medicine whilst not  always having the normal assurances that are associated with licensed forms.

This session provides an insight into some of the issues faced when procuring an unlicensed medicine, together with some recent examples , and some pointers that may provide some ‘peace of mind’ to pharmacists when dispensing unlicensed medicines.

KILLER QUESTIONS ON ENGAGEMENT WITH CUSTOMERS AND PATIENTS: A Special Presentation from Pfizer

As Pharmacists increasingly take on a frontline role with patients as practice-based service delivery becomes part and parcel of the busy community pharmacy, this session will examine some of the key issues that arise when engaging more closely with patients.

  • Challenges, e.g. proactively approaching an obese patient to offer a weight management service; discussing sexual health and erectile dysfunction, minor ailments

  • Dealing with a difficult clinical or medical question that you do not immediately know the answer to

  • Recruiting customers for MURs

  • Communicating unexpected bad news (e.g. a high vascular health risk score)

  • How to convince a customer who has challenged you about government targets rather than genuine patient care, that patient safety and health outcomes are the core objectives for you professionally.

  • How to convince a customer that you may be able to impart some valuable information about their medicines use, that the GP has not already dealt with.
