The rationale of developing stronger links between Primary and Secondary Care
Patient care pathways are becoming more complex and involving a growing number of professionals. The communication between different stakeholders is paramount for the continuity of care and a good patient experience. Important links between providers are highlighted on the communications between primary and secondary settings, for example upon transfer of care in the form of discharge information. It has been demonstrated that a significant part of these pieces of communication do not follow recommendations entirely, leading to the occurrence of medication errors. Nevertheless there are good examples than can improve practice and patient outcomes.
