Beyond GLP-1: The Weight-Management Conversation Pharmacy Is Missing

GLP-1 receptor agonists have changed the conversation around weight management, and pharmacy teams are on the front line of it. But the rise of GLP-1s has created a set of questions that a prescription alone doesn't answer. What do you recommend to a patient who doesn't want GLP-1 treatment, or isn't eligible for it? What role does pharmacy play once a patient starts a GLP-1 course, or after they come off one? And where does an accessible, non-prescription option fit alongside a drug that can cost over £500 every three months?

These are the questions Certmedica's Thomas Wingenfeld and Dr. Irmgard van Rensen will address in their Pharmacy Show theatre session, "Thinking beyond GLP-1: the role of formoline L112 in modern weight management." The session looks at where a long-established, non-prescription option sits in a landscape that has, for the past two years, been dominated by a single class of injectable drug.

The fat binder without the side-effect baggage

Every pharmacist knows the fat-blocking category — and knows why it stalled. The lipase inhibitor, dispensed for years on prescription and sold in a lower dose over the counter, is as well known for its gastrointestinal side effects as for its results. That side-effect profile is what limits how readily pharmacists recommend it, and how long patients stay on it.

formoline L112 EXTRA works differently. It does not inhibit intestinal lipase. Taken with a meal, it binds a portion of the dietary fat in the digestive tract, and that bound fat passes through the body unabsorbed rather than being digested and stored. The distinction is not academic: formoline L112 has been tested head-to-head in a clinical comparison against that established lipase-inhibiting drug, and it is the tolerability difference that separates the two approaches. For pharmacy teams, that means a fat-binding option that can be recommended without the side-effect discussion that has held the category back.

Twelve years of success in Germany — and over 20 million packs to prove it

formoline L112 arrives in the UK with its case already made. For 12 years in Germany it has been the country's leading weight-loss product, with more than 20 million packs sold — a success record built in one of Europe's most demanding pharmacy markets, long before GLP-1s changed the conversation. Sterimax, the UK's exclusive distributor, is now bringing that proven product to UK pharmacy. It is a Class III medical device, available over the counter, without the prescribing, monitoring or supply constraints that come with GLP-1 medicines, and it is intended to be used alongside a calorie-controlled diet and increased physical activity, not as a replacement for either.

Proven over 12 months, not 12 weeks

The core trial behind formoline L112 ran for a full 12 months: a double-blind, placebo-controlled randomised trial (Cornelli et al., 2017) involving 100 participants. Both the treatment and placebo groups followed a matched programme of reduced-calorie diet and increased activity — the trial measured what formoline L112 added on top of that programme, not against no intervention at all. Over 12 months, participants taking formoline L112 achieved 50% greater weight loss than those on placebo under those matched diet-and-activity conditions.

That distinction matters for pharmacy conversations: formoline L112 is not positioned as a stand-alone solution any more than a GLP-1 is expected to work without corresponding lifestyle change. It is positioned as an addition to the behavioural changes a patient is already being asked to make.

Beyond GLP-1, not against it

The session title is deliberate. "Beyond GLP-1" covers both of the roles formoline L112 can play: an option in its own right for patients who will not be taking a GLP-1, and ongoing support for those who do. For patients who are not candidates for a GLP-1, who are on a waiting list, or who have simply chosen that GLP-1 treatment is not for them, formoline L112 offers a pharmacy-accessible option with more than a decade of use behind it. For patients who do use a GLP-1, it offers pharmacy teams a lower-cost, non-prescription option to discuss for the periods before treatment starts or after it ends — including the point at which many patients come off GLP-1 treatment and look for ongoing support to prevent weight regain.
