How can neighbourhood health reduce health inequalities
Neighbourhood health offers a unique opportunity to tackle health inequalities by bringing care closer to communities and improving collaboration across primary care, community services, local authorities and the voluntary sector. This session will explore how community pharmacy can play a pivotal role within neighbourhood health models to improve prevention, early intervention and access to care for underserved populations.
Drawing on practical experience across seven North East London boroughs, Jignesh Patel will share insights from working with 37 Community Pharmacy Neighbourhood Leads and engaging with Integrated Neighbourhood Teams (INTs), operational boards, the Havering Integrator Group, PINT, and the South West Newham Cardiovascular Disease Steering Group. Attendees will learn how neighbourhood-based partnerships can address local population health needs, reduce unwarranted variation in outcomes, and strengthen community pharmacy’s contribution to reducing health inequalities.
