Pharmacy in care homes: the impact

Care home residents are among the most vulnerable users of medicines, often living with frailty, multiple long-term conditions and polypharmacy. This creates significant challenges in ensuring medicines remain safe, appropriate and aligned with residents’ individual needs and goals.

This presentation will explore the impact and evolution of pharmacy services in care homes, from the development of Medicines Optimisation in Care Homes (MOCH) and the integration of pharmacy professionals within multidisciplinary teams, through to the establishment of the National Pharmacy Care Home Faculty. The Faculty provides a national platform to connect and support pharmacy professionals working across the care home sector, share expertise and good practice, develop professional capability, and help shape the future direction of pharmacy practice in care homes.

The session will highlight the contribution of pharmacists and pharmacy technicians to medication review, deprescribing, medicines management, workforce support and collaborative working. It will consider the impact on medication safety, appropriate prescribing and polypharmacy, medicines-related problems, person-centred care and use of NHS resources, while reflecting on how the National Pharmacy Care Home Faculty can strengthen professional networks and support the continued development and sustainability of pharmacy services for care home residents.
